Patient Care
Total Care Nursing Service is a patient advocate agency that is committed to providing the safest and highest quality care to our patients. For us, providing quality care is always at the forefront of what we do as a home health agency.
But the constant work we put in to improve our quality is close behind. Much of our patient care innovations are hidden behind the scenes; our processes.
Our internal processes are designed to be measured and improved. We have developed a robust continuous quality improvement program that, from case conference to utilization reviews, highlights potential problems and corrects them. It is a methodical approach that results in clear gains in our quality measurements.
Communication
Poor communication has historically been a problem for our industry; a problem that we are really focused on changing. We pioneered innovations like open off-hours conferences and providing evening and weekend extended hours to support to our clients. Even our mobile case managers are changing how we engage.
On any given case our staff communicates with hospital case managers, attending and primary care physicians, patients, caregivers, family members and insurance providers. It is imperative that we get everyone on the same page and exceptional communication is the only way to accomplishing this goal.
Sharing Information
Any organization can be easily overwhelmed by the sheer volume of information that streams through our doors on a daily basis. It's the reality of a healthcare system that is constantly and radically reshaping itself.
To combat the overload, our physicians are assigned an account representative that along with providing updates about your patients, can keep them abreast of any changing regulation that could affect their practice.
Working Together
The ideal relationship between a practice and a home health agency is that of healthcare partners. We are at our best as a cohesive unit; collaborating to ensure our patients receives a high level of care. But we are even better when we use our shared experiences as a springboard to continually improve.
